For-Profit Submission Requirements: 1.
Completed application 2. Latest CPA Audited Financials (audited
financials are not required, but the Insured must provide the underwriter with
a complete balance sheet and income statement showing revenue and expenses).
3. If a start-up company, the underwriter will need complete
Proforma Financials. Proforma Financials consist of a balance sheet, income and
cash flow statement which are designed to show future financial results.
Underwriters will want to know where the funding for the company is coming
from, the amount and the schedule of the capital infusion. 4. The
most recent list of Directors and Officers 5. Employee Handbook, if
the company will be purchasing Employment Practices Liability Coverage as well.
6. Complete Claims History
