Information:
-Current completed and signed
Acord Workers Compensation Application. The insureds FEIN # and NCCI ID #
(where applicable) must be shown on application.
-Latest 3 5 years of
currently valued (within 90 days of effective date) company loss runs from
previous / current carrier. Note: A "No Loss" letter signed by the
insured, on the insureds letterhead, is acceptable for risks generating
annual premiums below $5,000.
-Loss date must include a
detailed written description of all losses greater than $25,000. The
description must include: type of injury, cause of injury, current status of
claim, valued within 90 days of policy effective date.
-Historical audited premiums
and Experience Modifications corresponding to the Policy Year Loss Runs
-Most recent year-end or
interim audited financial statements to be provided within 30 days of effective
date. Note: Any risk requesting a deductible plan must provide financial
statements in advance of quoting.
-Any risk experiencing a
material change in operations during the policy period must submit a revised,
completed and signed Application.
